Please Include The Following Columns

• Snack Box Identify the type of snack box you are interested in. i.e. "Premium Gift Box", "Employee Appreciation Snack Pack", "Healthy Snack Gift - 8 Snacks"
• Recipient Company Leave blank if not sending to a company address
• Recipient First Name First Name of Recipient
• Recipient Last Name Last Name of Recipient
• Recipient Full Name Full Name of Recipient (Only use this field if you skipped the first and last name field)
• Shipping address Street Address including Apartment or Suite Number
• Shipping City City of Shipping Address
• Shipping State State of Shipping Address
• Shipping Zip Code Zip Code of Shipping Address
• Message Small Personalized Gift Message
• Dietary Restrictions Any Dietary Restrictions? i.e. Gluten Free, Vegan, Kosher, etc.
